School absence notes for K-12 vs college and university: what's different
The documentation process for a school absence works differently depending on the level of education, and getting the wrong type of note can cause unnecessary delays with your school's administration.
Elementary and high school (K-12): For students under 18, the parent or guardian typically submits the absence documentation on the student's behalf. Most Canadian public and private schools accept a physician-signed note confirming the dates of absence and general reason for the illness. The school principal or attendance office is usually the recipient. In most provinces, schools do not require detailed clinical information — a note confirming the student was seen by a physician and was medically unfit to attend is sufficient.
College and university: Post-secondary institutions in Canada have more formal documentation requirements. Most universities including the University of Toronto, University of British Columbia, McGill, University of Alberta, and Dalhousie have their own medical documentation forms or require that the absence note meet specific criteria to qualify for academic accommodation. Common requirements include the physician's name and license number, the date of the consultation, the dates of incapacity, and a statement confirming the student was unfit to attend academic activities. Some institutions, particularly for exam deferrals, require the note to be submitted within 24 to 48 hours of the missed assessment.

Getting a medical note for an exam deferral or missed assignment
Missing a scheduled exam or assignment deadline due to illness is one of the most stressful situations a student can face. Most Canadian universities and colleges have a formal deferral or late submission process that requires medical documentation and the deadlines to submit that documentation are tight.
Here is how the process typically works at Canadian post-secondary institutions:
Step 1 — Notify your professor or registrar immediately. Most institutions require you to notify the relevant academic office before or on the day of the missed assessment, not after. Waiting until you feel better to inform them can invalidate your deferral request regardless of the documentation you provide.
Step 2 — Obtain your medical note the same day. Many universities explicitly require that the note be dated on or around the day of the missed exam. Step 3 — Submit through the correct channel.
