Fitness-to-work certificate Canada — physician clearing employee for return to physical role

Your employer has cleared you to come back after an injury. But before you step on site, they need written confirmation from a physician that you are physically capable of doing the job safely.

That document is a fitness-to-work certificate. It is more specific than a standard return-to-work letter and more common than most employees expect. Here is who needs one, what it covers, and how to get it.

What a fitness-to-work certificate Canada employers require

A fitness-to-work certificate is a formal document signed by a licensed physician confirming that an employee is physically fit to perform the specific duties of their role. Unlike a general sick note, which confirms you were unwell, or a return-to-work letter, which confirms you can resume work generally, a fitness-to-work certificate addresses whether you are physically capable of the demands of a particular job.

The certificate is common in industries where physical capacity directly affects workplace safety — construction, healthcare, transportation, manufacturing, emergency services, and mining.

A valid fitness-to-work certificate includes:

  • Your name and the date of the physician's assessment

  • A statement that you are fit to perform your specific job duties

  • Any restrictions or limitations that apply during recovery, if relevant

  • The physician's full name, provincial license number, and signature

  • Official letterhead

The certificate does not include your diagnosis. Your medical condition is private under provincial privacy legislation, and your employer is not entitled to that information.

Who needs a fitness-to-work certificate in Canada

Fitness-to-work certificates are required in several situations:

After a workplace injury. Workers who have been off on a Workers' Compensation Board (WCB) claim — in Ontario through WSIB, in BC through WorkSafeBC, in Alberta through WCB Alberta — typically need a fitness-to-work certificate before returning to their role. The certificate confirms their condition no longer poses a safety risk.

After an extended illness. Employees in physically demanding roles who have been absent for more than two to three weeks due to illness may be asked to provide fitness clearance before returning. This is common in healthcare, where staff handle patients, and in construction, where physical demands are high.

Before starting certain jobs. Some employers in transportation, mining, aviation, and emergency services require pre-employment fitness assessments. A physician confirms the applicant meets the physical requirements of the role before they begin.

After surgery or injury unrelated to work. An employee who broke their arm outside of work, for example, may need physician clearance before returning to a role that involves heavy lifting or machinery operation.

For regulated industries. Transport Canada requires medical assessments for commercial drivers and airline crew. The Canadian Nuclear Safety Commission requires fitness assessments for workers in nuclear facilities.

Fitness-to-work vs return-to-work certificate

These two documents are related but serve different purposes.

A return-to-work certificate is a general clearance document confirming you are medically able to resume work from a specific date. It is used after any illness or injury and is the standard document most employers request.

A fitness-to-work certificate goes further. It addresses whether you are physically capable of the specific duties your role requires. A healthcare worker returning after a back injury needs more than general clearance — their employer needs confirmation they can safely lift patients, stand for extended periods, and perform the physical demands of their position.

In practice, many employers use the terms interchangeably. If your employer asks for a fitness-to-work certificate, confirm with them what specific information they need the document to address so the physician can prepare it correctly.

What happens if you return without clearance

Returning to a physically demanding role without a fitness-to-work certificate when one is required creates several risks:

  • Your employer can refuse to allow you to work until clearance is provided

  • If you are injured while working without medical clearance, your WCB or WSIB claim may be affected

  • Your employer's liability insurance may be impacted in the event of an incident

  • In federally regulated industries, returning without the required medical clearance may constitute a safety violation under the Canada Labour Code

Getting the certificate before you return protects both you and your employer.

How to get a fitness-to-work certificate the same day

For employees who need clearance quickly, Canada Medical Notes provides same-day fitness-to-work certificates prepared by licensed Canadian physicians. The process:

  • Submit a request form with your name, province, role, and the specific duties you are returning to

  • Include any relevant details about your condition and the nature of your recovery

  • A physician registered in your province reviews the submission and prepares the certificate

  • The completed document arrives in your inbox the same day on official letterhead with the physician's license number

If your employer also requires a standard return-to-work medical certificate in addition to fitness clearance, both can be requested at the same time.

Province-specific notes on fitness-to-work requirements

Ontario (WSIB). The Workplace Safety and Insurance Board in Ontario requires medical clearance for workers returning to modified or full duties after a compensable injury. The Form 8 — Functional Abilities Form — is the standard document used in Ontario workplaces and can be completed by any licensed physician.

British Columbia (WorkSafeBC). WorkSafeBC uses a Physician's Report form (Form 8) to document an injured worker's functional abilities. Employers use this to plan modified duties and confirm the worker is safe to return.

Alberta (WCB Alberta). Alberta's WCB process uses the Attending Physician's Report to document medical status and functional abilities. Employers in Alberta may also request their own fitness assessment form to be completed by the attending physician.

Frequently asked questions

Can any physician issue a fitness-to-work certificate in Canada?

Yes. Any licensed physician registered under a provincial medical college can prepare a fitness-to-work certificate. The certificate must include the physician's name, license number, and signature. Employers cannot require the certificate to come from a company-appointed physician unless your employment contract specifically provides for this.

Does the certificate need to mention my diagnosis?

No. The certificate addresses your functional abilities — what you can and cannot do — not your underlying diagnosis. Your medical condition is private under provincial privacy legislation. The employer is entitled to know whether you are fit for your duties, not why you were unfit.

What if my employer has a specific form?

If your employer or their insurer has a standard form that needs to be completed by a physician, Canada Medical Notes can assist. Include the form with your request and specify what fields need to be filled. The physician will complete it according to your employer's requirements.

How long is a fitness-to-work certificate valid?

There is no standard expiry period in Canadian law. The certificate confirms fitness as of the date of the assessment. If your condition changes significantly after the certificate is issued, your employer may request updated documentation.

Can I get both a return-to-work certificate and a fitness-to-work certificate?

Yes. Both documents can be requested through Canada Medical Notes at the same time. Some employers require both — a general return-to-work clearance and a specific fitness assessment for the physical demands of the role.
